- Form C – Notice of daily periods – work persons employed
- Form E – holidays are given to workers
- Form F – register of holidays and leave granted
- Form h – quarterly returns

Kerala Shops and Commercial Establishments (Amendment) Rules, 2022
The Government of Kerala on August 02, 2022, issued the Kerala Shops and Commercial Establishments (Amendment) Rules, 2022 to further amend the Kerala Shops and Commercial Establishments Rules, 1961.
The following amendment has been made:
- Rule 2E shall be omitted;
- Sub-rule (3) of rule 2G shall be omitted;
- Rule 12A shall be omitted;
- Form H shall be omitted.
