Non-payment of this contribution is a serious offence because it directly affects insurable employees. Hence, the maximum imprisonment for this offence is 3 years and the minimum is 6 months with Rs. 5,000 as a fine.

In this instance:
The Director (Medical) Delhi, ESIC Hospital Complex, 5th & 6th Floor, Basaidarapur, New Delhi-110015, invites applications from Hospitals, Diagnostic Centers and Blood Banks which are empanelled with CGHS and situated in the region of Delhi, Gurugram, Faridabad, Manesar, Sahibabad, Noida, G. Noida & Ghaziabad for empanelment for two years for providing Super[1]Speciality Services (tertiary care only) as per clause 14.7 of page no. 21 of referral policy, to ESIC Beneficiaries of ESIC Hospitals in Delhi (Basaidarapur, Jhilmil, Okhla and Rohini) and NCR (Faridabad, Gurugram, Manesar, Sahibabad and Noida). Interested HCOs may submit their application up to 31.12 2023 as per Annexure A1, A2, A3 and A4 whichever is applicable duly filled along with the required documents mentioned in the application form on page no. 96 to 121 of the referral policy.
